Now that my head is being replaced, how do you initiate the request for the extended warranty? Do you negotiate it with the dealer or do you have to contact GM directly? I just want to streamline this as much as possible.

Brach, I would say based on my experiences, discuss this 1st with your Service Manager and see what kind of response you get. Above all be nice at the dealer. If you get nowhere there, then go to GM..They now handle these on a "case by case basis" so I am not real sure where you will go from there. I'd think an extended engine/drivetrain warranty letter would be in order at least.

Start by calling Hummer customer service.
I called after I had been to the dealer and while they were ordering parts. Get a file number started ASAP.

Be nice to the person on the phone (the first call they will just be taking down your information) but let them know you would like a show of good faith from GM in extending the waranty. Say you have a new vehicle that now has hadmajor surgery (head removal) and you are concerned about the long term and that you want to be sure the problem has been fixed.

Good Luck

Well, the head was replaced and it seems to be running good. I had contacted GM customer service while it was being repaired and requested an extended powertrain warranty. I advised my service manager that I had contacted GM and he said he would step in and get me the warranty. I will keep my fingers crossed that this goes smoothly.
