Front Brake Pad Shims

Front Brake Pad Shims
Quick question. I just finished replacing front pads and rotors with GM factory parts. Included in the pad kit were two shims for each pad, one stainless steel and one slotted/teflon coated.
Our last brake job was done at a tire store and only lasted about 15k miles before nasty brake pulsations started. (it was mid-Winter and I was feeling accordingly lazy)
The pads I pulled out were Bendix installed without shims so I had nothing to work from for the installation. To play it safe I only used the stainless shims.
Anyone know if both shims are needed per pad and which shim should be installed closest to the pad?
Thanks!
