if I ever need to be higher than 1st when I'm in 4low, I start in that gear. I am driving a 5speed Adventure though. But there's so much torque at that incredibly geared down speed, I'll start in 3rd no problem. I never shift to a higher gear while on the move and in 4low, because I also get that hard shift you're talking about. Not sure if it's doing any potential damage, but just doesn't feel good so I avoid it.
